1008 Budget Amendment Request

This form must be completed by the Lead Applicant.
This document is necessary to ensure that any proposed changes from the original application are allowable and align with funding guidelines. Lead Applicants are asked to submit these requests so IDOE personnel may review and approve the amendments, and prevent potential future issues with reimbursement for expenses not in the accepted plan.

A change in budget is only necessary if the changes exceed 10% within a line item, or moves within budget line items.

The last date to submit an Amendment request will be 03/31/2022 for single-year and 04/15/2023 for multi-year grantees to ensure sufficient review time.
